Management of lower eyelid blepharoplasty complications is the challenging tasks for the eyelid surgeons. Most of complications are minor and transient and rarely major and permanent with functional and aesthetic consequences. The visual complications include partial or complete visual loss, hematoma or intraorbital hemorrhage, dry eyes, epiphora and chemosis. The most severe aesthetic complication is the malposition of the lower eyelid resulting in retraction, ectropion deformation of the lateral canthus and lagophthalmos. These malpositions are often minor, sometimes reversible, but they can be major, with psychological, aesthetic, and functional consequences. The understanding of the surgical anatomy, causes and pathophysiology of the problems and the evaluation of clinical findings are so important for choosing the choices of management for lower eyelid blepharoplasty complications. This presentation would reveal the systemic approach for evaluating patients that can assist the surgeon in choosing the surgical procedure(s) required to correct the lower eyelid complications with minimal complications.
